Southern Health Partners Inc is seeking a dedicated LPN for the Evening Shift (3pm to 11pm) to join our Healthcare & Medical Services team providing compassionate care within correctional facilities. In this role, you will deliver direct patient care to incarcerated adults in accordance with nursing standards, facility policies, and state regulations, helping to promote a safe, healthy environment. Working closely with RNs, physicians, mental health professionals, and custody staff, you will perform medication administration, treatments, and ordered procedures; obtain and document vital signs and health histories; assist with sick call and chronic care clinics; respond to medical emergencies; and support triage and assessment of patient needs. You will maintain accurate, timely documentation in medical records, ensure appropriate infection control and safety practices, and contribute to care plans that address both acute and chronic conditions. The position requires consistent, professional communication, respect, and discretion when working with incarcerated individuals, as well as strong teamwork and collaboration with facility personnel.
