Southern Health Partners Inc is seeking a dedicated LPN for the Evening Shift (3pm to 11pm) to provide high‑quality nursing care within a correctional healthcare setting. As part of our Healthcare & Medical Services team, you will deliver compassionate, evidence‑based care to incarcerated adults, supporting medical, dental, and psychiatric services in compliance with state regulations and facility policies. In this role, you will perform focused assessments, obtain vital signs, administer medications (including oral, IM, and other approved routes), complete treatments and wound care, collect specimens, and carry out provider orders accurately and safely. You will monitor patients for changes in condition, respond promptly to urgent and emergent situations, and collaborate closely with physicians, nurse practitioners, dentists, mental health providers, and custody staff to ensure continuity and safety of care. The LPN Evening Shift nurse will document all care in the electronic medical record, maintain accurate medication administration records, and follow established clinical protocols, infection control standards, and safety procedures specific to correctional environments. You will participate in intake screenings, sick call, chronic care clinics, and medication passes, while maintaining professional boundaries and security awareness at all times. Additional responsibilities include patient education on medications and health conditions, assisting with coordination of off‑site appointments or hospital transfers, and contributing to quality improvement and compliance efforts.
