Southern Health Partners Inc is seeking a dedicated LPN for our evening shift from 3:00 pm to 11:00 pm to provide compassionate, high-quality healthcare services within a correctional facility setting. As an LPN on our team, you will deliver direct nursing care to incarcerated individuals, including performing patient assessments, administering medications, providing treatments, dressing changes, and monitoring vital signs in accordance with physician orders and facility protocols. You will document all care accurately and timely in electronic medical records, assist with sick call and chronic care clinics, support physicians, nurse practitioners, and RNs during examinations and procedures, and respond appropriately to medical emergencies. In this role, you will educate patients on medications, treatment plans, and health maintenance, while maintaining strict adherence to safety, security, and infection control standards. You will collaborate closely with custody staff and other healthcare professionals to ensure a safe, organized, and respectful environment, contribute to care plans, and communicate changes in patient condition promptly.
