Southern Health Partners Inc is seeking a dedicated LPN for the Evening Shift (3pm to 11pm) to provide quality healthcare services within a correctional facility setting. As an Evening Shift LPN, you will deliver direct patient care to incarcerated individuals, including routine assessments, vital signs, medication administration, wound care, and assistance with chronic disease management, in strict accordance with physician orders, nursing protocols, and state regulations. You will respond to sick calls and urgent medical needs, accurately document all care in electronic and/or paper medical records, and support medical providers during exams, treatments, and minor procedures. Working closely with an interdisciplinary team, you will help maintain a safe, sanitary clinical environment, follow facility security procedures, and participate in infection control and quality improvement activities. In this role, you will provide patient education on medications, treatment plans, and health maintenance, demonstrate professional and compassionate communication with a diverse and often high‑need patient population, and collaborate with custody staff to coordinate clinic schedules and ensure safe movement of patients.
